• If you've gone through this website, downloaded and read the packet, and still have questions that prevent you from applying with us, you may request a one-on-one consultation with our program staff.  Please complete the form below. You will receive an email confirming date and time.

• If you are ready to apply, please submit an application below. Please note, there is a $350 non-refundable application fee that you will be electronically invoiced for after submitting your application.​​

• If you have already completed a Home Study with A Family Tree and are continuing services, please email hello@afamilytree.org for the Home Study Update (HSU) application

• Fees are for adopting parents only. Birth parents do not pay any fees to A Family Tree or the Ka Makana program. All services to birth parents are free.
