If you've gone through this website, downloaded and read the packet, and still have questions that prevent you from applying with us, you may request a one-on-one consultation with our program staff. You can fill out the form at the bottom of this page.

If you are ready to apply, please submit an application below. Please note, there is a $350 non-refundable application fee that you will be electronically invoiced for after submitting your application. A staff member will reach out to you to confirm receipt of the application.

If you have already completed a Home Study with A Family Tree and are continuing services, please email hello@afamilytree.org for the Home Study Update (HSU) application
In order to use the fillable fields function, you MUST open the document in the Adobe app. If you do not have the app, you may print out the application, complete it legibly by hand, and either upload a scanned copy below in PDF FORM, email it to hello@afamilytree.org, or mail it to 1632 S. King St, Honolulu, HI 96826. Please submit all additional forms in PDF form as well.
